Individual Tax Return — Information Form

JSA Accounting & Taxation

This form helps us get everything we need to prepare your tax return and make sure you claim everything you're entitled to. You don't need any tax knowledge to fill this in — just answer honestly and as best you can. If a question doesn't apply to you, simply skip it or select "No". Anything you're unsure about, we'll talk through together at your appointment.

1Your Details

🔒 For your security, please don't write your Tax File Number (TFN) on this form. You can provide it to us securely at your appointment or through our secure client portal.

Are you married, in a de facto relationship, or do you have a spouse for tax purposes?
This affects some offsets and Medicare levy calculations — it doesn't mean you lodge a joint return.

2Your Occupation

Tell us your main occupation and we'll highlight the work-related expenses that people in your line of work most commonly claim — you can still add anything else that applies to you further down.

3Income

Select everything that applies to you this year. You don't need exact figures for everything — for most items we just need the relevant statement or summary, which we'll ask you to bring or upload.

4Work-Related Expenses (Deductions)

These are expenses you paid for yourself that relate to earning your income. Tick anything you spent money on this year — a box will open for a few extra details. Not sure if something counts? Tick it anyway and we'll check for you.

5Tax Offsets, Rebates & Other Adjustments

These can reduce the tax you owe or affect your Medicare levy. Answer Yes/No — we'll only ask for more detail where it's relevant.

Send to JSA securely emails your completed form to us — you can still print or save it as a PDF if you prefer. Either way, please also bring or send the statements and documents mentioned in each section.
